OCFS-6000 - Packet - Required Forms and Clearance List

The OCFS-6000 Packet is the official Required Forms and Clearance List issued by the New York State Office of Children and Family Services (OCFS). This packet is used by child care programs, applicants, staff, volunteers, and household members who must complete a comprehensive background check before working or residing in a licensed, registered, or legally exempt child care setting.

This packet provides the complete set of documents, fingerprinting instructions, and regulatory requirements needed to meet all state and federal background check standards.


Who Must Use the OCFS-6000 Packet

The packet is required for:

  • Individuals seeking to work, live, or volunteer in a child care program

  • Group/Family Day Care household members age 18+

  • Legally Exempt providers, staff, volunteers, and household members

  • Anyone who has not completed a comprehensive background check in the last 5 years

  • Individuals who do not qualify for a fingerprint waiver


What’s Included in the OCFS-6000 Packet

The packet lists all required forms and explains how each one is used in the background check process. Depending on the program type, the following documents may be required:

  • LDSS-3370 – Statewide Central Register Database Check

  • OCFS-4930 – Request for Fingerprinting Services (IdentoGO)

  • OCFS-6001 – Information for Provider, Staff, Volunteer, or Household Member

  • OCFS-6002 – Qualifications (licensed/registered programs)

  • OCFS-6003 – References

  • OCFS-6004 – Medical Statement

  • OCFS-6005 – Criminal Conviction Statement

Background Checks Covered in This Packet

The packet supports completion of all the following checks:

  • New York State criminal history check (via fingerprinting)

  • National criminal background check (via FBI)

  • New York State Sex Offender Registry search

  • National Sex Offender Registry search

  • Statewide Central Register (SCR) child abuse and maltreatment check

  • Staff Exclusion List (SEL) check

  • Out-of-state child abuse, sex offender, and criminal history checks (for states lived in during the past 5 years)

The packet also includes the required FBI Privacy Act Statement.


Submission Process

For Licensed and Registered Programs

All documentation should be submitted electronically through the Facility Application and Management System (FAMS) using the program's MY.NY.GOV account.

For New Programs Applying for a License or Registration

The full OCFS-6000 Packet must be submitted directly to the program’s regulator.

For Legally Exempt Providers

All required forms must be submitted to the enrollment agency, and fingerprinting must be scheduled through IdentoGO.


Purpose of the Packet

The OCFS-6000 Packet ensures that all steps of the background check process are completed correctly. Once all forms and checks are processed, OCFS will issue written notification confirming whether the individual is approved to care for children.
